hep for pensioner with recent leg amputation

My visually impaired father has just had a leg amputation. At 83 his strength and resilience is beyond me. But we need help. 
We are struggling to get him into a specialist rehabilitation centre. No one is saying but I get the impression his age is keeping him low on the list.
Based in London -where is this kind of help available
He wants to get home and there is talk of a hospital bed for him and a wheelchair.
I don't know where to start to get help for a stairlift and any adaptations to the house.
My mother is his main carer but she herself is recovering from cancer treatment.
NHS seem to be trying to free the bed up (understandable) but without a proper care package in place we cant let that happen. 
any advice on how to get help with all this would help hugely right now

    Welcome to the community :)

    I'm really sorry to learn about your Father's recent leg amputation and the difficulties you're having getting him into a specialist rehabilitation unit.

    Please see the information on the NHS Choices website about care after discharge from hospital. Your Father's care needs should be carefully assessed before he is discharged. You're right that he can't go home without the right care package in place. Does your Father have a care plan?

    Age UK may have a local service to where your Father lives so it's worth getting in touch with them to see if they can offer advice and assistance maybe even advocacy.
