Can you give more than one answer per question or do you have to pick just one thankyou in advance

    You should answer each question as fully as possible.
    Can you give us a bit more info ?
    Is this a review form ?
    is it a PIP2 ?

    please give an example of a question that you are trying to answer and your thoughts as to an answer this may help to point you in the right direction

    First time for PIP on the DLA forms you had to pick one from say 4 possable answers is it the same for PIP?.I notice this is based on a points system highest being 15 on some 12 on others.I think i read somewhere you can pick 2 that could make the score higher .Thankyou CR For your all your help its appreciated

    Ok for PIP the highest is 12 points
    only the highest score counts in each descriptor you cannot add two togethet
    so for example

    4. Washing and bathing.
    4 a. Can wash and bathe unaided. 0 points
    4 b. Needs to use an aid or appliance to be able to wash or bathe. 2 points
    4 c. Needs supervision or prompting to be able to wash or bathe. 2 points
    4 d. Needs assistance to be able to wash either their hair or body below the waist. 2 points
    4 e. Needs assistance to be able to get in or out of a bath or shower. 3 points
    4 f. Needs assistance to be able to wash their body between the shoulders and waist. 4 points
    4 g. Cannot wash and bathe at all and needs another person to wash their entire body. 8 points

    you cannot add 4d and 4f only  the highest will be counted so 4f will be the applied descriptor + 4 points
