Yes, definitely apply for PIP. Ask your medical practitioners to issue you with a DS1500 form and send it with the PIP form.
There are normally Macmillan advisers associated with hospitals who can sort this kind of thing out for you, but if you want to do it yourself, you can just call PIP on 0800 917 2222 and get a form.
