Hi all,

I hope folk don't mind me posting on hetr I think that you might be well placed to help me as I collect data for the research section of my extended independent academic study, which will form part a part of my Access to Health Course.

This survey is about transitions from children's to adult services for young people with complex needs, a topic that is also close to my heart as I support a lot of young people with this move.

The survey should only take a few minutes, consists of a small number of multiple choice questions and is completely anonymous.

Your participation is very welcome. Please feel free to pass this link to any relevant individuals that you think might be interested - I'm particularly keen to hear from people working in health, education, social services and local government roles in the UK.

Many thanks for your time.
