Thanks to @Pippa_Scope for inviting me to join this conversation.
Knowing so little about your circumstances, functional abilities and what you have tried it is hard to know what to suggest.
It does sound as if you potentially need to be working with specialist rehabilitation health professionals. Occupational Therapists (OT's) are specialists in helping people to be able to regain the ability to be able to achieve the activities they needs to do, by breaking the task down into manageable steps and assessing if the introduction of aids or adaptations would help.
If you have never had rehab you will need your GP or consultant to refer you, specifically tell them that this is what you want. Assuming it is clinically appropriate they will refer you, but there may be a waiting list. Even if you have had rehab in the past you might still be able to access more, contact them again and ask if they think there might be more they can do to help you.

I have 2 disc dislodged in back and find when walking I get pain in lower abdomen feel numbish in left leg

presumably the MRI is to get a clearer idea of what is causing your difficulties and to help inform potential treatment options. However, If you are struggling with self care activities (such as washing, dressing, toileting, meal preparation etc ) then you can request the the Adult Social Care Team at your local authority do an assessment of your care needs and put services in place if appropriate.

Thanks Jean appreciate my last MRI scan showed 2 disc out not pressing nerves I'm very confused about why pain and nerve damage I'm ok with putting t shirts on etc lower parts can be problem learned a lot from pain management
