If this is for ESA this link may help. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form/Evidence will be needed to support your claim. No one can help you fill out the form on an internet forum unfortunately because no one knows how your conditions affect you.If you're still not sure what to do then try and search google to see else is near you, welfare rights maybe.
I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help.
