    HI,

I'm afraid it won't be possible to help you to do that on an internet forum. There's guides available to help you do that here.

Remember to add as much information about how your conditions affect you as possible and add 2-3 examples of what happened that last time you attempted that activity for each descriptor that applies to you. Use extra paper if you need to but remember to add you name and NI number to each piece. Sending in any relevant evidence you have to support your claim with the form because they rarely contact anyone.

If you're still unable to fill them in yourself then you'll need to contact an advice centre near you. This link will help you find what's near you.

I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
If i see a question that i know the answer to i will try my best to help.
