what does this mean even tho the points counted up show 8 points for mobility 8 for standard care

That's the box where the HCP has to write a justification for the descriptor choice. As there's a recommendation for 8 points then the box should not be ticked. Underneath that there should be reasons why that descriptor has been recommended.

Maybe a case of the left hand no knowing what the right hand is doing? Mind you this is typical of such a report - complete nonsense.
