Hi.  What information do you guys put in it? I had an SEN and a disabled students allowance assessment which provides evidence of needing a P.A to do note taking and help with manual dexterity tasks. I had 50% extra time for exams and assignments, extensions and rest breaks due to fatigue and stamina issues. I'm wondering if it's worth including this as extra information, but not sure how to link it in with the descriptors or if it can be linked?

    Thanks. Does it matter if I haven't got the reports, as it was a long time ago.  It's on my summary record that I had an SEN but not the report. Is it enough then to say what help I received?

    I have evidence of needing extensions, rest breaks due to fatigue and stamina issues for exams and assignments and how the work too significantly longer.  This also applies to most activities of daily living, so is it enough to write that ?

    All you can do is send what you and write as much information as possible about how your conditions affect you. Adding a couple of real life examples of what happened the last time you attempted that activity for each descriptor that applies to you.
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
