Return address confusion

Hi,sorry if been asked before.
I received my esa50 questionaire that needed to be returned by 6th feb 2020.They included an envelope with return address wolverhmapton.
Today i received a reminder letter to return by 6th feb,however it says return to address at too of letter.
Which ia health center birmingham,different to the envelope address sent out with form..
Where do i return form and med proof too?

    HI,

    Use the envelope that was provided with the ESA50 form. You do need to make sure it's received by that date because there's no extension of time for returning this form.
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.

    Hello,, I don't mean to be rude but why wait till this late hour to ask on an internee forum your question, when y cold have rang the DWP and got your answer shortly after receiving the letter and you saw noticed the difference in addresses.

    Hi wilko,i was waiting on paperwork from my cardiac specialist ,if i  had i would of sent via envelope but whilst waiing received the reminder.which just confused me.

    @deeds1970 as a community champion here on scope, i've helped thousands of people here on the community. Please don't worry about asking these types of questions. Lots of people have really bad anxiety and are unable to ring to ask these types of questions and this is what were are here for and it's not a problem to answer any questions you have.

    I live with my daughter and thankfully she has me but because of her anxiety, Autism and learning disability she's unable to use the phone, even for those that she knows. I'd like to think that if she didn't  have me then she could at least turn to a public forum like this for any questions she has.
    I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying.
