Eligible for income based ESA but worried about form

I have received a letter from DWP saying I could be eligible to go from contribution based to income based esa.  I’m concerned if I provide my partners income details I maybe worse off. Just confused by the whole situation not sure if to complete the form or not. Anyone else have a partner who works over 24 hours as it states on the form.

    Great to meet you and welcome to our community. My understanding is that you only have to complete the form if you want to be re-assessed. However, I'm no expert (yet) and we have lots of knowledgeable members who will want to advise!

    If your partner worked more than 24 hours per week for the entire time then you will not qualify
    There should be letter with the form, you should tick the relevant box and return in the envelope provided you do not need to complete the form

    I've gone over the letter and form and there is no tick box for any of the 3 criteria.

    Being sent a form from the DWP, despite that the enclosed letter says I do not need to fill out the form,  I'm worried that if I don't do something then I've done something wrong.  I dont understand why they are sending forms out when they do not need to be returned.  I feel like I'm being trapped or tricked somehow. 

    So I've circled the relevant criteria on the front of the form and signed and dated it.

    It doesn't feel right not to return it.  I'm worried sick over this ?
