North East London Mobility Assessment Centre - Your views needed!!

Hello,
I am working on a project funded by the Department for Transport to establish if there is a need for a mobility assessment centre in North East London, particularly Tower Hamlets, Hackney and Newham as these areas do not currently have easy access to an assessment centre.
Mobility assessment centres traditionally offer assessments for disabled people looking at outdoor mobility, this includes driving adaptations, scooter and wheelchair assessments, however there is a move towards offering transport advice in the local area as an additional service.
To determine what sort of service gets put in this area I need people's views! So if this is of interest to you please follow the link below to a questionnaire. The questionnaire is focused towards service users, however carers are also welcome to complete please just comment that you are a carer in one of the comments boxes.
Any additional thoughts/comments via this forum will also be welcomed as I'm trying to capture as many views as possible.
