i had decided to go to Tribunal, but not sure how to go about this. Do I ask DWP's for a form or do I write to someone else?

If you already have had a mandatory reconsideration (MR) decision letter it will tell you on this to download a tribunal appeal form from HMCTS website. The DWP will have sent you an extra copy of their decision letter to submit with your appeal.
Disability Rights UK site (DR) has a good guide to PIP appeals. -
@charlene, I take it your MR wasn’t successful?
I used a template online to write my appeal letter.
I think there maybe one on this site somewhere.
It was easier as you answer the questions online and at the end a letter appears, I did add things to it though.

Thanks everyone for your help. I was awarded enhance care, but only 4 points for mobility. I did send of a doctors letter which confirmed that I walk very slow, and that in 2014 had an ultra sound on my knees showed substantive arthritis. Doctor when on to say that my quality of life and how far I can walk could be affected., which it is. Received this after the MR, but sent it to DWP anyway. What is the chance DWP will change their mind at this stage?
BTW, I have managed to buy a car, so thankfully I am still mobile and not a prisoner in my own homeWhen the going gets tough, the tough get going. -

28 days from date of MR decision letter. But you can appeal within 13 months if you give a reason for late appeal.

Wow thought I was about to have a heart attack then. Still staying to myself deep breathes. My MR reconsideration letter is dated 2nd May!When the going gets tough, the tough get going.

For now you only have to give an outline of your grounds for appeal and you can make your full submission when you receive the DWP bundle from the tribunal.
Send appeal form to tribunal by recorded delivery. -

Contact the 'Welfare Rights officer' in the Council. Make an appointment and ask them to help you fill the form in and represent you at the tribunal aswell.
Tell them that you are disabled and you feel you are also being institutionally bullied by the Atos Health Assesor and DWP aswell...!
The Welfare Rights officers get paid by the Councils to help the genuine disabled people's appeals process.
