Freedom Pass mobility assessment

I have recently applied for a disabled person's Freedom Pass, and have been invited for a mobility assessment later this month. Has anyone here had one of these, and can give me some idea of what to expect? Most of my issues travelling (aside from constant tremors) are distance-related, so I'm not sure how they're going to be able to test that!

Cassandra, this is much too late to help you I'm afraid, but I have recently acquired a copy of the mobility assessment form (not for applicant's use, rather for the assessors) which was sent to me by a member on Atos Miracles. It needs to be shared, for the benefit of others, so feel free to download.
