What part of the form are you are you having problems with? -
I'm just not good at these forms at all. It's the What Are You're Disabilities and how do they affect you. It is going to take me a year to put it all down on paper and it's so hard to find the words to describe it

I know what you mean I had the same problem when I first started. The best thing I found is for somebody to sit there and help you i e close friend or relative. Also if you have your original notes diagnosing your condition You can use them to refer to. Tell them absolutely everything about your condition even feel sorry for yourself while you're writing it this probably sounds difficult but give it a go
