Help - Composite Wheel Lock Research

Hi, I'm new to the forum so hello to everyone out there.
I'm doing some research into the failure rates of composite wheel locks used on many chairs, TiLite, etc. There are a few manufactures as well.
If you've had a composite wheel lock fail (it is usually the spring that breaks) could you let me know the following so that I can add it to the information I have?
Make of Wheelchair
Make of Wheel Lock
Length of time in use until failure
I have had three failures in three years. Other than the spring breaking the wheel locks are OK.
